Sourcing note: The $204/$360/$128 fee figures are confirmed only on the Board's own page -- HAR Chapter 71 defers the actual renewal/restoration fee amounts to a separate fee-schedule chapter (16-53) that could not be located at a working URL. What triggers lapsed status: Licenses and permits renew biennially by December 31 of odd-numbered years. Per the Board's own page, licenses not renewed by the expiration date are unlicensed and may not practice.
